    Most recent survey dec 2932:...24. Yes 24 violations and 2 immediate jeopardies. Keep your loved ones safe and away from this abysmal place. Mismanaged by an out of state mining company (Bedrock) that bought this and 8 other homes while they were in receivership. They should get the hell out of a business they know nothing about.

    The recent 2022 state survey: 24 violations 2 immediate jeopardies. A mining company with no real knowledge of patient care owns this and has mismanaged it to the point of patient harm. Please keep your love ones safe and AWAY from here.
