    The facility is not clean period. The smell of piss hits you as soon as you walk into the door. 40 year old carpet in main area. There was not real separation of people for therapy & long term residents. They did not have bed set up properly. Therapist only come during the week apparently. The only thing remotely nice about the place was the staff & only reason for 2 stars they were welcoming & seemed nice. Wheeled my father in Wheeled him right back out. Grayson County should should be ashamed of having 60+ resident in that unclean, old run down environment for their seniors.
